  • Harbor Intranet Main page
    • "iMedConsent Informed Consent"
      • login: your e number
      • password: your password
  • Search for form
  • Click "Select patient"
    • Enter MRN, choose the correct patient and click "Go"
  • Fill out form
  • Click "Generate Document"
  • Printer function should pop up and click "print"
