- Ampicillin 50mg/kg IV q6hrs PLUS
- Cefotaxime 50mg/kg IV q6hrs OR Gentamicin 2.5mg/kg IV q8hrs
- If suspecting S. pneumoniae or MRSA, add Vancomycin
