Radial head subluxation

Pathophysiology
Transverse tear in annular ligament with entrapment of annular ligament between radial head and capitellum
Diagnosis
- Age 1y-5y (peak 2y-3y) d/t swinging pt by arm
- Sudden onset
- Absence of edema, focal tenderness, or bruising
- ANY swelling/bruising NOT typical so XR!
Treatment
- Pronation alone (93% successful)
- Extend elbow then quickly supinate & flex (84%)
- if back in child will move arm again in <30 min
