Hematocele
Background
- A collection of blood outside of the testicle within the scrotal sac (in the tunica vaginalis)
- Most commonly caused by trauma or surgery
Clinical Features
- Recent blunt GU trauma
- Absence of fever
- Scrotal swelling
- Scrotal pain
Differential Diagnosis
- Pyocele
- Hydrocele
- Spermatocele
- Testicular torsion
- Testicular malignancy
- Orchitis
- Epididymitis
- Fournier's gangrene
- Testicular rupture
- Scrotal wall hematoma
Evaluation
- Scrotal ultrasound with doppler
- Positive cremasteric reflex
- Poor transillumination of scrotal sac
Management
- Urology consultation if related to acute trauma
Disposition
- If isolated hematocele, likely outpatient follow-up
