- For DHS-empaneled, message their DHS provider to f/u on the lab
- Route LAB Follow-Up HAR, Reason Lab/Radiology Follow-Up
- Use only for Adult ED patients to follow outpatient COVID tests, urine cultures and STI tests
- Route CCC-HAR, Reason Lab/Radiology Follow-Up
- Use for non DHS-empaneled Adult ED patients’ lab tests other than above
- CCC reviews requests in a few business days during business hours
- Be specific when filling out the details in the CCC box
- eg, Follow up on TSH results, and adjust levothyroxine dosage as needed
- CCC will only call patient if an abnormal result needs to be addressed.
- If you think a patient needs an urgent face to face evaluation, have them go to the Urgent Care if DHS eligible.
